Your First Appointment at Abilene Artificial Limb!

At Abilene Artificial Limb, we understand your first appointment with our team may seem overwhelming. Our mission is to provide a calm and welcoming experience to everyone who visits our office. With our initial appointments and evaluations free of charge, we welcome to assist you. Whether you are a preoperative prosthetic consultation or currently a prosthetic patient with another provider, during your first appointment, our team will conduct a thorough examination, recommend an appropriate prosthesis and begin the fitting process. What to Do Before Your First Appointment 


Before arriving at Abilene Artificial Limb, below is a list of items that need to be completed before your first appointment: 

  • New amputees and patients must receive approval or a release to be fitted with a prothesis. To obtain this, ask for a prescription for prosthesis.
  • Those experienced with prosthetics must contact their physician and discuss weight gain or loss or any pain or discomfort they are currently dealing with from the prosthesis. Ask your physician for a letter of medical necessity that includes these problems and ask for a new prothesis prescription.
  • All prosthetic patients will need to contact one or more of the following organizations per your preferred method of financial assistance or insurance. These include your insurance company, workers compensation carrier, employer or adjuster, and managed car authorization representative. 

What Should I Bring to My First Appointment?


1.     Active Insurance Cards

a.     Providing critical data for billing purposes, your insurance card provides policy number, group numbers, effective dates, plan codes, co-pays, deductibles, physician phone numbers, referral/authoriation information, and additional information. These cars help our team identify your coverage amongst the different insurance plans.

b.     If you are not the subscriber to the insurance plan, please let our team at Abilene Artificial Limb know at the time of your appointment (i.e. if you are covered on your spouse’s insurance).

2.     Prescription or Referral

3.     Photo ID

4.     Payment for any associated service costs, such as Deductibles, Co-Insurance, and Deposits.
